What are some of the mistakes made by inexperienced doctors?

Our JR2 got cold, cough, fatigue, bilateral cervical lymphadenopathy and on evaluation was found to have raised LFT.

Rest of the routine work including cultures and viral markers were negative.

Now, what.

Yeh, monospot is positive

Eureka, EBV associated lymphoma

(though PCR negative lol ! )

Let’s confirm on biopsy! Ok done

Long case discussion short: Resident was diagnosed as Hep E positive with reactivation of EBV, no malignancy.

Lesson: Especially, for young lads: Diagnosing a rare disease excites every Osler, but missing the most common diseases (endemic ones) in co-existence in the same patient and at same time is very common.
